Medicare beneficiaries. With the original
$500M allocated, CCTP focuses on the
following objectives:
?Improving transitions of beneficiaries
from the inpatient hospital setting to
other care settings;
?Improving quality of care;
?Reducing readmissions for high-risk
Medicare fee-for-service (FFS)
beneficiaries; and
?Documenting measurable savings to
the Medicare program.
