Main Menu - This is main menu form where contain three buttons. Button 'Pendaftaran' is to display regisration form. Button 'Disediakan Oleh' is to display form about us and button 'Keluar' is to exit from the system.
Registration - This is registration form where the customer who want to get the health card have to register before they ca get their card. Button 'tambah' is to add new customer. Button 'Kemaskini' is to display update information form. Button 'Cetak' is to print health card. Button 'Kembali' is to go back to Main Menu.
Update Information - This is form that can update the customer information. By click to the button 'Kemaskini', user can edit the customers informations details. Button 'Laporan' is to display form report and can been print.
Print Health Card - This is a Health Card form that print the specific health card after the customer register. The user has to print the card according to the customer specific customer ID
Print Report - This is a Report that will display all the information of customers details that has been register. This form also can be printed as hard copy if needed.
About Us - This is the form about the designer. This system has been develop by 2 people that is Mr. Mohd Faris Daud from University Technology Mara and Mr. Zul Fakhri Zolkefli from University Kuala Lumpur.
Health Card - This is just example of health card that will printed from the system. This card show the information about the customer and this card is very important to the customer who want to work around Kuantan town.
Login - Login form for the useer to enter username(Nama Pengguna) and password(Kata Laluan) before can enter the system
